  Instructions:

  1. Bring both your original vehicle title and driver license.
  2. If name change is due to divorce, also bring the decree or court order showing name change. (certified copy is acceptable)
  3. If name change is due to marriage, also bring the marriage certificate. (certified copy is acceptable)
  4. If vehicle is titled in multiple names, all owners will need to appear.
